Municipal/City Social Welfare and Development Office
The Medical Assistance Program of City of Ilagan extends financial help to indigent patients confined in government or private hospitals, or those needing maintenance medicines, laboratory procedures, chemotherapy, or dialysis.
Assistance is released either as a guarantee letter to partner hospitals or through direct payment to the pharmacy or provider.
Who is eligible
- Bona fide resident of the locality
- Assessed as indigent or financially incapacitated by the Social Welfare Office
Requirements
- Certificate of Indigency
- Medical abstract or clinical certificate (within 3 months)
- Hospital bill / statement of account or doctor's prescription
- Valid ID of patient and representative
How to apply
- Proceed to the Social Welfare and Development Office assistance desk.
- Submit complete requirements for assessment.
- Undergo interview by the social worker.
- Receive guarantee letter or assistance per schedule of release.
